I'm April Ekeh, from Colorado. 

Member ONS: The Denver Metro Oncology Society, Pikes Peak Oncology Nursing Society and this year ONS Congress is in Denver for their 50th year!

Do you all have regional Oncology societies also?

I have looked at other nursing specialties and Oncology seems to be very well supported. All the venues we have for education elevates our profession.

Hello, 

We ran out of time yesterday to review the questions for Metabolic Emergencies. The speaker. Christina Klein, kindly answered the questions here (with citations! ooh, ahhh): 

Q1: Use of compression stocking with active DVT proximal placement? Explain this more. They do not allow SCD with positive DVT on inpatients. Should we be using the stockings and SCDs? 

I was referring to using graduated elastic stockings not sequential compression devices. Compression stockings are not contraindication in venous thromboembolic conditions. They are used to reduce pain and swelling associated with DVT which can help the patient to ambulate thus preventing further development of thrombosis (Shah, et al., 2021, Shrye, et al., 2016).

Q2: DDIMER - is it very much the go-to lab? If elevated then yes to DVT? Or is an elevated DDIMER only an element? 

D-Dimer is often checked 1st & if elevated an ultrasound or doppler study is done to detect DVT. D-Dimer is elevated because endogenous fibrinolysis is occurring in the body to counter the development of thrombosis (Shah, et al., 2021).

Q3: With the DIC, how long does it take for the reaction to resolve? We are supporting organs until this resolves - what does that look like? 

This time it takes from DIC to resolve varies for every patient. We treat the underlying cause & utilize techniques to provide adequate tissue perfusion to vital organs such as oxygen, IV fluid, blood products, and possibly mechanical ventilation & vasopressors, treat bleeding with RBC, platelet, Fresh Frozen Plasma or Cryoprecipitate, & treat thrombosis with prophylactic anticoagulation when appropriate (Levi, 2019, Papageorgiou, et, al., 2018).

Q4: In regards to SIADH - please expound on free water restriction. 

Free water restriction refers to oral intake, a fluid restriction component. For fluid restriction in SIADH, we limit the patient’s oral intake of water as well as any parenteral fluid administration. Educate the patient on how much water they can drink daily (Shepshelovich, et al., 2017).

I wanted to see if any ambulatory sites were working on quality improvement projects and which ones you are working on. We have wanted to work on a CLABSI project but find it hard to get statistics being outpatient
